11 Intestinal malabsorption, rather than inadequate dietary intake, can explain most cases of vitamin B12 deficiency (5). Absorption of vitamin B12 from food requires normal function of the stomach, pancreas, and small intestine. Stomach acid and enzymes free vitamin B12 from food, allowing it to bind to R-protein (also known as transcobalamin-1 or haptocorrin), found in saliva and gastric fluids. In the alkaline environment of the small intestine, R-proteins are degraded by pancreatic enzymes, freeing vitamin B12 to bind to intrinsic factor (IF), a protein secreted by specialized cells in the stomach. Receptors on the surface of the ileum (final part of the small intestine) take up the IF-B12 complex only in the presence of calcium, which is supplied by the pancreas (5). Vitamin B12 can also be absorbed by passive diffusion, but this process is very inefficient—only about 1% absorption of the vitamin B12 dose is absorbed passively (2). The prevalent causes of vitamin B12 deficiency are (1) an autoimmune condition known as pernicious anemia, and (2) a disorder called food-bound vitamin B12 malabsorption. Both conditions have been associated with a chronic inflammatory disease of the stomach known as atrophic gastritis.

The third clinical question is whether cobalamin therapy is indicated. Finding either IF-related malabsorption, which suggests a risk for progression, or signs suggesting mild but clinical deficiency justifies management as clinical deficiency. Patients suspected of FBCM require oral doses of 1000 μg daily, preferably on an empty stomach.39–42 However, a nonmalabsorbing, asymptomatic patient with a lone, mild biochemical abnormality can be safely deferred for cobalamin or MMA reassessment a year later.

If you’re serious about the whey protein powders you use, then you probably have already analyzed Body Fortress Whey Protein (BF) powders. I’ve been weightlifting and using powders for about 40 years – my favorite is Optimum Nutrition Gold Standard (ON Gold). The GNC store closest to my house where I buy ON Gold moved, so I took a look at . 1. The protein ‘per scoop’ of BF is higher than most powders, because the scoop is about 30% bigger, so you pay more per gram of protein for BF as compared to many other brands. The front package label shows ’60g Protein’ – that’s 2 scoops; 2. BF has a lower ratio of whey isolates as ingredients (isolates are higher ‘quality’ protein); 3. The taste is fine. Compared to my favorite ON Gold, ON Gold costs more per pound, BUT I only need to mix about 75% of what would be equivalent to BF. And, the other nutritional content is ON Gold is superior, per serving, to BF. High quality whey protein powders are worth the money; BF is not a good value.

Whey protein hydrolysate – WPH is considered to be the “predigested” form of whey protein as it has already undergone partial hydrolysis – a process necessary for the body to absorb protein. WPH doesn’t require as much digestion as the other two forms of whey protein.

Ascorbic acid can be created in the laboratory much less expensively (and of course much more profitably). Scientists discovered that they could take corn syrup, mix it with hydrochloric acid, and voila: ascorbic acid! (By the way, the corn is likely to be genetically modified and not organically grown.)
